Seksualiteitsbeleving blijft een controversieel gegeven, waarin verlangens en angsten voor een impasse kunnen zorgen, maar ook voor een doorbraak. De behandeling ervan balanceert op deze dunne grens. Dat biedt, zeker bij eetstoornispatiënten met hun achtergrond, levenservaringen en persoonlijkheidsstructuur, boeiende perspectieven én ernstige beperkingen. Het valt op hoe schaars zowel literatuur als onderzoek over deze thematiek is. We treffen telkens dezelfde onderzoeks- en therapiecentra aan. Opvallend daarbij is hoe weinig de seksuele opvoeding aan bod komt, hoewel het belang hiervan onmiskenbaar is (zie Balakrishna & Crisp 1998). Dit blijft een onontgonnen terrein dat meer vragen oproept dan antwoorden geeft.

De grote afwezigen in dit hoofdstuk zijn de mannelijke patiënten. Zelf hebben we te weinig ervaring met deze groep en ook de literatuur is nog erg mager (zie Andersen, Cohn & Holbrook 2000). Maar ook in andere opzichten is onze blik selectief geweest. Ons schrijven over het verband tussen eetstoornissen en seksualiteit wordt onvermijdelijk bepaald door de huidige sociaal-culturele context. Dit geldt bijvoorbeeld voor het 'hyperfeminiene' rolpatroon dat leidt tot de paradoxale situatie waarnaar Tuiten c.s. (1993) verwijzen (zie § 1).

Een ander actueel thema is seksueel misbruik. Het onderzoek naar de gevolgen van misbruik in de kindertijd is sterk verbeterd. Het is niet langer een simpele oorzaak-gevolgredenering. De aandacht voor de tussenliggende variabelen heeft ertoe geleid dat we meer oog hebben voor 'beschermende' invloeden en 'gezonde' krachten, zodat we de betrokken patiënten vanuit een positievere gezichtshoek bekijken dan vanuit de slachtofferpositie. Dit heeft positieve gevolgen gehad voor de behandeling. Toch missen we in dit verband specifiek onderzoek naar preventie van eetstoornissen na traumatiserende ervaringen (zie ook hoofdstuk 17).
